Comparative study of 5 days letrozole therapy versus 10 days letrozole therapy for ovulation induction in infertile women.

Abstract: Background: Letrozole, which is an aromatase inhibitor is a newer ovulation induction agent. There still are many queries regarding the ideal protocol of letrozole(LTZ) to be used in ovulation induction. In the present study we compare the radiologic and hormonal characteristics of two different dosage schedules of letrozole. We aim to compare the ovulation rates achieved with short course letrozole therapy which is given as 5 mg for 5 days and extended course letrozole therapy given 2.5 mg for 10days. Objectives:The objective of our study is to observe the ovulation rates achieved with short course of letrozole therapy and extended course of letrozole therapy in woman with primary infertility due to anovulation. Methodology: An Observational study including 120 infertile women who fulfill our inclusion criteria will be taken in the study. The participants will be allocated into two groups. The patients in group A will receive 5 milligram of tablet letrozole for 5 days starting from day 3 to day 7 of menstrual cycle and the patients in group B will receive 2.5 milligram of tablet letrozole from the first day of cycle to the tenth day. Patients will be followed up with serial transvaginal ultrasonography to note their mean day of ovulation, ovulation pattern, endometrial thickness.The patients will be treated for three cycles. Results: The number of patients that are ovulating will be greater in the patients taking extended regimen. The aggregate of number of follicles observed during stimulation will be higher in the patients taking extended regimen. The total number of follicles reaching a size of 14 mm and 18 mm will be higher in the extended letrozole group. Difference in endometrial thickness at the time of hCG administration between the two groups is not expected to be significant. Conclusion: The extended letrozole regimen produces more mature follicles and subsequently result in higher pregnancy rates compared to short course letrozole regimen.
