Sequencing and Phylogenetic of Dihydropteroate synthase (DHPS) gene in Pneumocystis jirovecii isolated from immunocompromised patients in Iraq

Abstract: Background: Pneumocystis jiroveci infections can cause pneumocystis pneumonia (PCP) or lead to colonization without signs of PCP. P.jirovecii dihydropteroate synthase (DHPS) gene mutations association with prior sulfa prophylaxis failure has been reported from both developed and developing countries. Over the years, different genotypes of P. jiroveci have been discovered. Objectives: in this study, we conducted to determine the prevalence of P. jirovecii genotypes in immunocompetent and immunocompromised patients in Iraq and determine the clinical relevance of these detected mutations. Detection of P. jiroveciicysts was performed by Toluidine O blue (TOB) stain and Diff-Quik stain (DQS) stain. DNA detection was performed by polymerase chain reaction (PCR) using primers for the major surface glycoprotein (msg) gene. Samples positive for msg were genotyped based on the dihydropteroate synthase (DHPS) genes. Results: out of 79 immunocompromised patients, 24 revealed the presence of P. jiroveci DNA. Of 24 cases, ten (12.7%) were positive by TOB staining, four (6.3%) were positive by Diff-Quik (DQS) staining, and all 24 were positive by msg PCR. DHPS mutations were observed in 19(24.1%) of immunocomromised patients studied.
