The Prophylactic Effect of an Extract from Fermented Egyptian Mango (Mangifera indica L.) Peel Against Cisplatin-induced Cardiotoxicity and Nephrotoxicity in Rats

Abstract: Cisplatin (CP) is an effective anticancer drug for various types of cancers. Cardiotoxicity and nephrotoxicity are the side effects that are most limiting in its use. It was hypothesized that Egyptian mango (Mangifera indica L.) peel extract, shown to have a protective role against such toxicity, could have a stronger effect if fermentation were engaged. Fermentation is a process for disposing of globally problematic accumulations of agro-peels. This study evaluated whether solid-state fermentation using Kluyveromyces marxianus NRRL Y-8281 yeast on Egyptian mango (Mangifera indica L.) peel extract would enhance its protective role against cisplatin-induced-toxicity in rats. The experiment used 36 rats divided among 6 groups: the control group, cisplatin group (CP), groups treated with unfermented (UFMPE) and fermented (FMPE) mango peel extracts, and cisplatin treatment plus the extracts, labeled the CP-UFMPE and CP-FMPE groups. Cisplatin injection was shown to result in elevation of the activities of transaminases (AST and ALT), creatine kinase (CK), and lactate dehydrogenase (LDH) and the levels of troponin I, urea, creatinine, malondialdehyde (MDA), and tumor necrosis factor alpha (TNF-a), along with a decrease in total antioxidant capacity and in reduced glutathione (GSH). Treatment with UFMPE and FMPE ameliorated all the biochemical and histopathological damage caused by cisplatin. The improvement was more potent in the CP-FMPE than in the CP-UFMPE group. Based on these findings, the fermentation of mango peel increased its prophylactic effect against cisplatin-induced heart and kidney toxicity.
